Job Title: Patient Care Navigator

Position Summary
We are seeking a compassionate and detail-oriented Patient Care Navigator to join our healthcare team. This individual will play a vital role in supporting patients through all aspects of their care journey—helping them schedule appointments, access community resources, and overcome barriers that impact their health and well-being. The ideal candidate is proactive, organized, and deeply committed to ensuring every patient receives the support they need to achieve positive outcomes.
Key Responsibilities
- Schedule patient appointments, follow-ups, and diagnostic tests.
- Ensure completion of referrals, imaging, and specialty visits.
- Track and follow up on missed appointments or delayed care.
- Coordinate transportation and interpreter services as needed.
- Reinforce clinician instructions to promote adherence to care plans.
- Identify and connect patients with community resources such as housing, food assistance, and transportation programs.
- Refer patients to social work, behavioral health, case management, or financial counseling as appropriate.
- Help patients express their questions and concerns to the care team.
- Ensure patient values and preferences are integrated into care decisions.
- Support patients in overcoming barriers such as language, financial challenges, or lack of health literacy.
- Assist with insurance-related tasks, including prior authorizations and coverage verification.
- Document all encounters and care coordination activities in the EMR.
- Communicate patient updates and needs to physicians, nurses, and other team members.
- Conduct reminder calls for appointments or preventive health screenings.
- Engage high-risk or disengaged patients to re-establish care.
Qualifications
- Education: High School Diploma or equivalent required.
- Certification: Graduate of an approved Medical Assistant program.
- Experience: Previous experience in a medical setting or at least 2 years of relevant work experience preferred.
- Skills:
- Strong communication and interpersonal skills.
- Bilingual preferred.
- Proficient in basic computer technology and electronic medical records.
- Must successfully complete organizational orientation and training requirements.
